Rhode Island Court Forms

You can find court forms on the Rhode Island Judiciary website.  Depending on your relationship with the abuser, you may need to file in district or family court - the forms are listed separately for each court.  If you aren’t sure which court to file in, go to Am I eligible? Do I file in family court or district court? for more information.

You can also go to the courthouse to ask the clerk for the forms. Click on Rhode Island Courthouse Locations to find the courthouse in your area.  A local domestic violence organization may be able to provide you with the necessary forms as well.  To find help in your area, go to Rhode Island Advocates and Shelters.
